BLENDED LEARNING SURVEY & WAITLIST
For students who are currently Distance Learning and would like to switch to our Blended Learning Option instead. The Blended Learning option entails attending school two days a week and Distance Learning the other three days of the week.

This survey is only to be completed for students who are currently Distance Learning and would like to switch to our Blended Learning Option instead. The Blended Learning option entails attending school two days a week and Distance Learning the other three days of the week.
Completing this survey does not mean that your student will be placed in an in-person classroom on a Blended Learning schedule. This form will enter your student on our waitlist and as spaces become available for in-person instruction. You will be communicated with directly by an administrator if a spot for in-person learning becomes available for your student.
