For incoming Kindergarten and 7th Graders
Incoming Kindergarten and 7th Graders must complete a physical exam and immunizations before the start of the 2024-2025 school year. 6th Grade sent Form-14s home with your keiki recently. You can take this with you to your PCP or use their forms and bring it back.
